Hemostasis and sealing and Quill® in thoracic surgery by Corza Medical

Discover the far-reaching potential, features and benefits of a hemostasis and sealant patch and Quill® in thoracic procedures.

Is there a medical need?

In thoracic surgery, there is great need for sealing the lung tissue.

A hemostasis and sealant patch can be used to seal air leakages in the lungs (e.g. Marta et al. 2010 and Lang et al. 2004) and for hemostasis.

Options in thoracic surgery for using a hemostasis and sealant patch and Quill® for hemostasis and sealing1

The most important surgical procedures and areas of application in thoracic surgery for using a hemostasis and sealant patch and Quill®:

  • Lobectomies
  • Pulmonary resections (especially for atypical resections)

EXAMPLE 1

Reduction of the intensity and duration of air leakages

  • Randomised, prospective, open-label, multicentric trial
  • Efficacy and safety of sealant patch in a standard lobectomy
  • 189 patients after a standard lobectomy for lung cancer
  • After primary haemostasis using manual sutures or stapler, randomised to
    • Sealant patch group: n = 96
    • Control group (additional manual sutures): n = 93
  • Primary endpoint:
    • Air leakages 48 h after the operation
  • Secondary endpoints:
    • Reduction of the intensity of intraoperative air leakages after the initial test treatment
    • Intensity and duration of postoperative leakages Duration of the postoperative thorax drainage up to 9 days after the operation

Average duration of postoperative air leakages

Case studies for thoracic surgery

Corza salant patch in upper lobe resection due to lung cancer

Support of hemostasis and reinforcing the staple line.

Preliminary remarks

  • If there is an isolated tumour in the lungs, the indication for resection can be made.
  • The operation with curative intent is the procedure of choice in stages I, II and IIIA.1
  • The recommended surgical procedure is lobectomy including systematic dissection of the ipsilateral lymph nodes.1
  • The average 30-day lethality for open resections is 3.5% and the average 30-day morbidity is 30%.1
  • One postoperative risk is secondary bleeding, as after the lungs are re-aerated, the pressure on the (staple) sutures is increased, thus increasing the risk of suture insufficiency.
  • Corza sealant patch adhesion is an additional option for sufficient hemostasis and reinforcing the (staples) lines.

Patient history

  • 65-year-old patient with lung cancer in the right upper lobe (ICD-10 C34.1) (secondary tumour).
  • Known hepatic metastasis of a neuroendocrine tumour (NET), most likely in the jejunum first diagnosed in 2005.
  • Additional secondary findings: CHD, arterial hypertension, diabetes mellitus, obesity
  • No increased risk of bleeding.
  • Due to the size of the tumour, indication for resection of the right upper lobe.
1: Resected upper lobe of the lung.
2: Intraoperative site after resection of the upper lobe. Closure of the branches of the pulmonary artery with staples.
